Rectal biopsy as an aid to cancer control in ulcerative colitis.

چکیده

There is general agreement that malignant change in ulcerative colitis mostly occurs in those patients whose large intestine is totally involved and have a history of symptoms for more than 10 years. Having thus defined the population of colitics most at risk the problem remains of identifying the individual patient destined to get carcinoma. There is yet no evidence that all the population at risk will eventually develop malignant change. It has been previously pointed out that some test is required which will decide that a patient with ulcerative colitis has entered a precancerous phase (Morson, 1966). The detection of epithelial changes suggestive of precancer or carcinoma in situ in rectal biopsies from patients with ulcerative colitis prompted an investigation into the incidence and extent of such changes in colectomy specimens removed for colitis with and without malignant change. The results of this enquiry are reported here as well as an analysis of the first nine patients in whom the diagnosis of precancer was made in a rectal biopsy and played an important part, together with clinical and radiological observations, in the subsequent decision to perform total proctocolectomy.

Diagnostic difficulty arising from rectal recovery in ulcerative colitis.

AIMS To ascertain whether the dogma that a normal rectal biopsy precludes a diagnosis of ulcerative colitis is correct. METHODS Rectal biopsy specimens from a prospective group of 24 asymptomatic patients, with an established diagnosis of ulcerative colitis, were examined in a blinded study alongside 10 normal rectal biopsy specimens from an age and sex matched patient cohort without ulcerati...

Sigmoidoscopy and cytology in the detection of microscopic disease of the rectal mucosa in ulcerative colitis.

EDITORIAL SYNOPSIS This study confirms the value of both the sigmoidoscopic assessment and the cytology of the mucosal secretion in the detection of microscopic disease of the rectal mucosa in ulcerative colitis. On certain occasions, however, microscopic disease is undetected using these methods, so that rectal biopsy is recommended as an aid in the diagnosis of patients with a history suggest...

Rectal biopsy and precancer in ulcerative colitis.

Forty-seven patients with ulcerative colitis subjected to elective proctocolectomy were investigated with regard to the occurrence of precancerous changes in the rectal biopsy and the operation specimen. Thiol methyltransferase activity in inflammatory bowel disease.

BACKGROUND Luminal anionic sulphide may contribute to epithelial damage in ulcerative colitis. Thiol methyltransferase (TMT) governs sulphide detoxification by the colonic mucosa and circulating erythrocytes. AIMS To measure levels of TMT activity in erythrocytes of surgically treated cases of colitis or in rectal biopsies of defined groups of colitis. PATIENTS Venepuncture blood was obtain...

Value of sigmoidoscopy and biopsy in detection of carcinoma and premalignant change in ulcerative colitis.

Of 111 carcinomas developing in 73 patients with ulcerative colitis, 46 (41.5%) arose in the rectum where they are directly accessible to sigmoidoscopy. Fifty-eight per cent of single carcinomas developed in the rectum. The extent and frequency of rectal dysplasia was assessed by examining slides of rectal mucosa with an eyepiece micrometer. Slides from 46 patients with carcinoma and 22 patient...
